WHAT DOES THIS 30 MIN HEALTH CHECK CONSIST OF?

During your appointment the following will be completed:

Hearing and Vision screening
Blood pressure
BMI
Cholesterol and Blood Glucose

IMPORTANT INFORMATION

You will be required to sign a consent form regards the following information below.

You will be able to received a copy of your health screening results, and any other protected health information that is collected by Auckland Workplace Health, in connection with your health screenings (test results will be emailed to given address in the booking).

The test results will not be used by Auckland Workplace Health for purposes of individual health information or to make a diagnosis of any disease or illness. Auckland Workplace Health does not practice medicine and is not a substitute for your doctor’s care.
